摘要:
乳糜胸是由不同病因导致胸导管破裂或阻塞,继而引起乳糜进入胸腔的疾病,保守治疗失败时通常需要手术干预。鉴于不同猫的胸导管解剖结构差异大,在胸导管结扎前对猫胸导管及其分支进行造影成像是外科计划中必不可少的辅助手段。随着计算机断层扫描(computed tomography,CT)成像技术在犬猫淋巴管造影中的逐步应用,各种CT淋巴管造影方法被提出。本文汇总了目前猫乳糜胸诊疗中的CT淋巴管造影方案,以供临床参考。

与图 6同一只患猫,行CT淋巴管造影检查。A:矢状面最大密度投影(MIP)可见胸导管及乳糜池显影(箭头)(骨窗,窗位:300 HU,窗宽:1500)。B:俯卧位横断面第3胸椎水平,可见左侧胸腔内腹侧高密度沉积物(箭头),考虑造影剂泄露沉积于胸腔内重力侧(纵隔窗,窗位:40 HU,窗宽:400)。C:俯卧位横断面第12胸椎水平可见主动脉背侧的胸导管分支呈高密度(箭头)。术前扫描有助于分辨胸导管走行及其分支,协助制定手术计划(纵隔窗,窗位:40 HU,窗宽:400)。D:胸导管结扎术后行CT淋巴管造影检查,矢状面最大密度投影(MIP)可见胸导管内高密度影像达L2椎体水平,远端未见显影,提示结扎成功。T11-T12水平椎体腹侧多个高密度影像(箭头)为手术钛夹(骨窗,窗位:300 HU,窗宽:1 500)"
